As the vaccine has a limited shelf life, please arrive promptly for your appointment. When you arrive please follow the instructions on the message board on our porch



Availability
 
Perry Physicians currently has a supply of the MODERNA COVID-19 vaccine available and is vaccinating all persons age 18 and over, starting April 13, 2021. 
Perry Physicians will offer Moderna Covid Vaccine Boosters beginning October 25, 2021. 2nd Moderna Covid Vaccine Boosters are available starting Monday, April 4, 2022.


 
Scheduling
 
First vaccine appointments will be opened on a rolling basis as vaccine becomes available. You may call 717.582.2090, choose option 9. Second vaccine appointments are scheduled at the time of the first vaccine injection. In some circumstances a third vaccine dose, one month after the second dose is needed. This is for immunocompromised persons. If you believe you need a third dose for the initial series, please contact the office to discuss. First Covid Boosters may be scheduled 5 months after the administration of the final vaccine of the original series. The second Covid Booster can be scheduled 4 months after the first Covid booster.


A scheduled vaccine appointment is preferred to receive the vaccine, to assure availability of vaccine and staff!

Booster eligibility
  • All persons over 18 years of age are currently eligible for a Moderna booster, 5 months after the last dose of the original series.
  • All persons over 50 years of age are currently eligible for a second Moderna booster, 4 months after the first booster
  • All persons over 12 years of age who are moderately or severely immunocompromised are eligible for a second covid booster 4 months after the 1st booster (Pfizer only for 12-17 year olds)
Been receiving active cancer treatment for tumors or cancers of the blood
Received an organ transplant and are taking medicine to suppress the immune system
Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
Advanced or untreated HIV infection
Active treatment with high-dose corticosteroids or other drugs that may suppress their immune response
  • Persons over age 12 are eligible for a Pfizer booster, which is not available in our office.
  • Patients 18 years of age and older may have a Moderna booster, regardless of which vaccine was given as the original series.
 
Preparing for Your Appointment

 
Once you are scheduled, please print and complete the COVID Registration form and the Vaccination Checklist. You must bring the forms with you to your appointment. The links to the forms are found below. Please review our HIPAA Statement, the Emergency Use Authorization and What to Expect after getting a COVIC-19 Vaccine.

Cost
 
There is no charge or out of pocket cost for patients for the vaccine or administration. The vaccine is provided by the federal government. The administration fees will be paid by medicare, medicaid or your insurance. Please bring your insurance card or your Blue and Red Medicare Card to your appointment. There will not be a copay, coinsurance or deductible associated with this service. Persons without insurance will not be charged for vaccine administration.
